How are payments handled?
As a courtesy to our patients, we will bill insurance for all office visits and procedures. We ask that you pay any portion not covered by your insurance due to deductible or copayments on the day of service.
What is your appointment cancellation policy?
Should you need to cancel your appointment, please give 24 hour prior notice in consideration to other patients. Failure of 24 notification will result in a $25 fee.
Do you offer telemedicine or virtual visits?
Due to the current Covid-19 pandemic, we do offer the convenience to see the doctor from the comfort of your home via a secure telemedicine site.

What to bring to my appointment?
Please bring all insurance cards, a valid government issued ID, and method of payment. We accept cash, check, or Visa/Mastercard.
What if I am late to my appointment?
Out of respect for all our patients, we may ask you to reschedule if more than 15 minutes late.
Do you offer lab services in your office?
For the convenience of our patients, laboratory services are provided by CPL in our office. Please note you will be billed separately for any labs ordered.
