What insurance do you accept?
We accept Aetna, Allwell, Ambetter, Amerigroup, BCBS, Community Health Choice, Humana, Medicare, Memorial Hermann, Molina, Superior Health, Tri West, UHC Community, UHC Commercial, and UHC Medicare. This is not intended to be a complete list or a guarantee of coverage. Please check with your insurance provider to confirm that we are in network.
How are payments handled?
As a courtesy to our patients, we bill insurance for all office visits and procedures. We ask that you pay any portion not covered by your insurance, such as your deductible or copay, on the day of service.
What forms of payment do you accept?
We accept cash, check, Visa, and Mastercard.
Who do I contact with billing questions?
We are proud to partner with Express Medical Billing. Please call them directly at 1-800-873-2559, extension 1038, with any questions about your bill.
What is your appointment cancellation policy?
If you need to cancel your appointment, please give at least 24 hours’ notice as a courtesy to other patients. Failure to provide 24 hours’ notice will result in a $25 fee.
What happens if I am late to my appointment?
Out of respect for all of our patients, we may ask you to reschedule if you arrive more than 15 minutes late.
What should I bring to my appointment?
Please bring all insurance cards, a valid government-issued ID, a method of payment, and a list of the medications, vitamins, and supplements you currently take.
Do you offer lab services in your office?
Yes. For the convenience of our patients, laboratory services are provided by CPL in our office. Please note that you will be billed separately for any labs ordered.
Are you accepting new patients?
Yes. Nabeel Khan, MD, is always accepting new patients, whether you are sick, managing a chronic condition, or due for a regular checkup.
Where can I find new patient forms?
New patient forms are available on our Online Forms page. You can download and print them, then fax your completed forms to our office or bring them to your appointment.
What are your office hours?
Our office is open Monday through Thursday from 8:30 AM to 5:00 PM, closed for lunch from 12:00 PM to 1:00 PM, and Friday from 8:30 AM to 12:00 PM. We are closed Saturday and Sunday.

What does an internal medicine physician do?
An internal medicine physician, or internist, specializes in the prevention, diagnosis, and treatment of health conditions in adults, from routine preventive care to the management of complex chronic illnesses.
Can Dr. Khan be my primary care physician?
Yes. Dr. Khan provides primary care for adults, including annual physical exams, preventive screenings, and ongoing management of chronic conditions.
What conditions do you treat?
We diagnose and treat a wide range of adult health conditions, including arthritis, asthma, diabetes, heart disease, high blood pressure, high cholesterol, and thyroid disorders.
How often should I have a physical exam?
Most adults should have a physical exam once a year. Annual exams help identify health problems like high blood pressure, high cholesterol, and diabetes before they cause symptoms.
Do I need to fast before my blood work?
Some blood tests, such as cholesterol and glucose tests, may require fasting beforehand. Our team will let you know in advance if you need to fast.
Do you coordinate care with specialists?
Yes. When your care requires specialized expertise, Dr. Khan coordinates with specialists such as cardiologists to ensure your treatment stays connected.
