A little preparation goes a long way toward making your first appointment smooth and productive. To save time at check-in, we encourage you to download and complete our new patient forms before your visit. You can fax your completed forms to our office or bring them with you on the day of your appointment.
Please remember to bring the following items with you:
- All current insurance cards
- A valid government-issued photo ID
- A complete list of the medications, vitamins, and supplements you take
- Copies of any previous medical records, lab results, or imaging you’d like us to review
- A method of payment for any copay or deductible (we accept cash, check, Visa, and Mastercard)
- A list of questions or health concerns you’d like to discuss
What to Expect at Your Appointment
When you arrive, our front desk team will check you in and verify your insurance information. A clinical team member will then take your vital signs, including your height, weight, blood pressure, and blood oxygen level.
Next, you’ll meet with Dr. Khan in a private exam room. He will review your medical history, talk with you about your lifestyle and any current symptoms, and perform an examination. If you need blood work, it can often be completed right in our building, since laboratory services are provided on-site by CPL.
At the end of your visit, Dr. Khan will talk with you about his findings and recommendations. If you have a chronic condition such as diabetes or high blood pressure, he will work with you to build a personalized management plan and schedule follow-up care as needed.
Insurance and Payment
As a courtesy to our patients, we bill insurance for all office visits and procedures. Any portion not covered by your insurance, such as your deductible or copay, is due on the day of service. We accept many major insurance plans, including Medicare. Appointment Policies
Out of respect for all of our patients, we ask that you arrive a few minutes early for your appointment. If you arrive more than 15 minutes late, we may need to reschedule your visit. If you need to cancel, please give us at least 24 hours’ notice so that we can offer your appointment time to another patient.

Where can I find new patient forms?
Our new patient forms are available on our Online Forms page. You can download and print them, then fax your completed forms to our office or bring them with you to your appointment.
What should I bring to my first appointment?
Please bring all insurance cards, a valid government-issued ID, a method of payment, a list of your current medications, and any previous medical records you would like us to review.
What forms of payment do you accept?
We accept cash, check, Visa, and Mastercard. Any portion of your visit not covered by insurance, such as a copay or deductible, is due on the day of service.
Do you accept Medicare?
Yes. We accept Medicare along with many other major insurance plans. Because coverage can vary, please check with your insurance provider to confirm that we are in network.
What happens if I am late for my appointment?
Out of respect for all of our patients, we may ask you to reschedule if you arrive more than 15 minutes late.
What is your cancellation policy?
If you need to cancel your appointment, please give at least 24 hours’ notice. Failure to provide 24 hours’ notice will result in a $25 fee.
